Personal Information Protection Policy

Health and Welfare Information Association

With regard to the handling of personal information, including specific personal information, Health and Welfare Information Association (hereinafter referred to as the “Association”) hereby establishes the following policy on the protection of personal information and ensures appropriate handling thereof.

1. Compliance with Laws and Regulations

The Association will comply with the Act on the Protection of Personal Information, the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures and other applicable laws and regulations and ensure a proper execution of business.

2. Proper Collection of Personal Information

The Association will collect personal information in a lawful and proper manner.

3. Use of Personal Information

The Association will use personal information within the scope of the purpose of use thereof.

4. Provision of Personal Information to Third Party

The Association will not provide personal information to any third party without the prior consent of the owner thereof unless otherwise permitted by applicable laws and regulations. The Association will not provide specific personal information to any third party with or without the consent of the owner thereof unless otherwise permitted by applicable laws and regulations.
The Association may outsource a part of its operations where necessary and provide a part of personal information retained by it for a contractor with which it has signed a confidentiality agreement. Even in such case, the Association will exercise appropriate oversight of the contractor.

5. Appropriate Management of Personal Information

The Association will ensure as much as possible that it retains accurate and current personal information necessary to fulfill the purpose of use thereof.
The Association will take adequate measures to prevent any leakage, loss or damage of the personal information and ensure safe and secure management thereof.

6. Disclosure, Correction, Addition, Deletion, Suspension of Use of Personal Information

If the owner of personal information retained by the Association makes a request for disclosure, correction, addition, deletion or suspension of use thereof or any other request therefor, the Association will promptly respond to such request according to prescribed procedures. As regards specific personal information, however, the Association will appropriately and promptly delete or dispose of the information once the purpose of use thereof is achieved and its retention period required under applicable laws and regulations has elapsed.

7. Response to Complaint Regarding Handling of Personal Information

The Association will appropriately and promptly respond to any complaint made regarding its handling of personal information.

8. Establishment of System and Provision of Training to Personnel to Protect Personal Information

The Association will ensure the establishment of an appropriate management system and make efforts to raise the awareness of the personnel in order to protect personal information.

9. Purpose of Use of Personal Information Retained by the Association

(1) Purpose of use of personal information excluding specific personal information

The purpose of use of personal information in the Association is to improve health and welfare of the public through the appropriate implementation of the following operations:

(i) Operations concerning announcement, receptions of various events, administration in relation to projects organized by the Association, and

(ii) Production of books, magazines and in-house magazines and other publications issued by the Association, related purchasing and delivery management, provision of related information, etc.

(2) Purpose of use of specific personal information

The purposes of use of specific personal information in the Association are as follows:

(i) Clerical affairs related to individual numbers of those other than officers and employees of the Association

(a) Clerical work to prepare payment reports of remunerations, fees, etc.,

(b) Clerical work to prepare payment reports of real property lease, etc., and

(c) Clerical work to prepare payment reports of a remuneration arising from acquisition of a real property, etc.

(ii) Clerical affairs related to individual numbers of officers and employees of the Association (including their dependent family members)

(a) Clerical affairs related to withholding at source
